In the aftermath of the big dust-up at the end of the last episode, Sam and Dean wind up in Sioux Falls Hospital - a lair of the Leviathans - but are quickly saved by the arrival of Bobby, who spirits them away in a stolen ambulance (call me paranoid, but I found it a bit strange how easily the Winchesters accepted Bobby's escape from the fire at his salvage yard).
They hide out in the country, while Dean's broken leg heals. Sam claims to be okay, and admits that although he's still having hallucinations he can now tell what's real and what isn't.
However, leaving Dean slumbering in front of the TV (which is playing a trailer for My Bloody Valentine 3D - a horror flick starring Jensen Ackles), Sam slips out, taking the Impala, having seen a newspaper report about a murder that brings back memories of a case he worked as a kid.
Young Sam (Colin Ford) had been tracking a kitsune, a pituitary gland-devouring monster, and while doing so met a girl called Amy (Emma Grabinsky) - his first love(?), albeit fleeting.
Now, years later, he meets up with her again. Firefly's Jewel Staite plays grown-up Amy Pond (Doctor Who much?) and her connection to the murders and the killer's motivation is all sadly a bit predictable.
Everything's well acted and there's great drama and character interaction, all ably directed by Jensen Ackles, but there's no real surprises - even the climax with Dean (which has shades of the aftermath of the battle between Beatrix Kiddo and Vernita Green in Kill Bill) isn't totally unexpected.
It also seems rather strange at times, even though young Sam talks about his dad and Dean, that - because of time constraints etc - they never actually physically appear on screen and Sam is operating all on his lonesome.
Andrew Dabb and Daniel Loflin's untold tale of Sam's past, sandwiched into the on-going Leviathan's storyline, feels a bit light-weight, hurried and formulaic and not up to Supernatural's usual high standard.
